Hemimedullary syndrome
Findings
MRI showed diffusion restriction in left PICA territory and left medulla ( involving both medial and lateral medulla) sparing small portion of anterior medulla Diagnosis of hemimedullary syndrome was made on the basis of clinical features and imaging findings.
Discussion
Hemimedullary syndrome Aka reinhold syndrome Occurs as a result of occlusion of ipsilateral vertebral artery proximal to PICA and the anterior spinal artery branches Causes simultanoeus infarction of medial and lateral medulla and PICA territory Presenting features of lateral medullary syndrome and contralateral hemiplegia Another distinct clinical entity is Babinski Negiotte syndrome in which hypoglossal nerve palsy occurs concurrent to the above clinico radiologic picture.
