Answer for BIR CoW 03 Jan 2021
Ependymal (Neuroepithelial) Cyst
Findings
1) There is an Ependymal (Neuroepithelial) Cyst expanding the posterior horn of the left lateral ventricle.
2) There is no blockage of CSF flow or evidence of raised intracranial pressure, therefore this is likely to be an incidental finding.
Discussion
Differential Diagnoses:
1. Choroid Plexus Cyst
2. Arachnoid Cyst
3. Epidermoid Cyst
4. Asymmetric ventricles
Image Interpretation Pearls
• Nonenhancing thin-walled cyst with CSF density/intensity on all MR sequences.
• Intraventricular common, typically lateral ventricle Pathology
This is a rare congenital benign cyst lined with ependymal cells.
They are thought to develop from sequestration of developing neuroectoderm. Clinical Presentation in young adults (<40yrs).
Typically asymptomatic, however may present with symptoms of raised intracranial pressure, headache, seizure.
Treatment If symptomatic, surgical excision or decompression – rapidly cures symptoms. Conservative treatment if asymptomatic.
