Answer for CoW 06 November 2016
CVT WITH ICH
Findings
Plain CT shows linear intracerebral hemorrhage with surrounding oedema in left frontal lobe. follow up contrast ct shows increase in both size of hemorrhage and surrounding oedema. filling defect is seen in superior sagittal sinus.
Discussion
An intra-cerebral haemorrhage, or intra-parenchymal cerebral haemorrhage, is a subset of an intracranial haemorrhage. There is acute accumulation of blood in the parenchyma of the brain. Cerebral venous thrombosis (CVT) refers to occlusion of venous channels in the cranial cavity, including dural venous thrombosis, cortical vein thrombosis and deep cerebral vein thrombosis. Superior sagittal sinus thrombosis (SSST) is the most common type of dural venous sinus thrombosis and is potentially devastating. The empty delta sign is CT sign of dural venous sinus thrombosis of the superior sagittal sinus, where contrast outlines a triangular filling defect (clot). It is only described with CECT-scan or MRI. In the emergency setting CVT with intacerebral hemorrhage can be diagnosed with cect brain .
