Answer:
Bartholin `s Gland cyst
Findings:
Very large well circumscribed cystic lesion is noted in the posterolateral inferior third of vagina and introitus, abutting the labia on right side. The lesion is below the level of inferior aspect of Pubic Symphysis . It appear hyperintense in both T1W1 and FS T1W1 and relatively hypointense in T2W1. The location and the signal characteristics are typical of Bartholin`s gland cyst with Internal Mucinous /Hemorrhagic Content and Inflammation.
Discussion:
Bartholin cyst, is typically located more posteriorly and caudally, in the posterolateral introitus and medial to the labia minora. In contrast , Mullerian or Gartner`s cysts are located in the anterolateral vagina. Bartholin gland cysts are typically retention cysts from chronic inflammation, which leads to ductal obstruction from pus or thick mucus within the Bartholin glands. In a bartholin cyst varying contents of mucin and hemorrhagemay lead to spontaneous high signal intensity on T1. Whenever superimposed infection occurs, it presents as air-fluid levels within the cyst and an irregular rim enhancement on CT or MRI.
