Case Of the Week (COW) 13 Apr 2014
Answer:
ENDOMETRIAL POLYP PROBABLY MALIGNANT.
Findings:
Uterus shows thickened endometrium.ET 17 mm.
Cystic areas are seen in the endometrium.
COLOUR DOPPLER showed FEEDING VESSEL SIGN positive for endometrial polyp.
In addition to this low resistanse flow is seen in the sub endometrial and endometrial arteries.
Discussion:
DD for thickened endometrium
1. ENDOMETRIAL HYPERPLASIA
2. ENDOMETRIAL POLYP
3. ENDOMETRIAL CARCINOMA
Although cystic changes within the thickened endometrium are more frequently seen in benign condition they can also be seen in endometrial carcinoma.
Normal thickness of post menopausal endometrium is 5 mm or less.
In our case ET was 17 mm and small cysts were seen in endometrium with positive FEEDIND VESSEL SIGN and increased vascularity in the sub endometrial and endometrial vessels.
But the role of doppler in diagnosis of endometrial carcinoma is still controversial.
