Answer:
CA UNCINATE PROCESS WITH HEPATIC METASTASES
Findings:
Heterogeneously enhancing mass lesion noted in the region of uncinate process of pancreas compressing the distal common bile duct and main pancreatic duct. The lesion shows increased uptake in FDG PET SCAN. Pancreatic duct appears mildly dilated, measures ~ 4.9 mm. Common bile duct appears dilated and measures ~ 9-10 mm Multiple noncalcific left paraaortic and peripancreatic mildly enhancing nodes noted . Stable numerous moderately enhancing hypodense nodules of varying sizes noted in both lobes of liver, largest in the right lobe segment V / VI.
Discussion:
This was a proved case of carcinoma of uncnate process of pancreas . Fdg pet scan revealed increase in uptake in the primary lesion and liver metastases. It is useful in the followup of these patients to evaluate whethe they are stable or show progression. Ca19-9 is a valuable tool in patients with carcinoma of the uncinate process, its sensitivity has been documented at 90.4 percent4 and its specificity at 96 percent.
