Findings:
Upper Oesophageal Web
Mucosal irregularity in lower third of esophagus.
Discussion:
Many of you would have spotted the mucosal irregularity in lower third of esophagus, which with the history of dysphagia would indicate a carcinoma esophagus. But the important finding is the oesophageal web, which is well seen in the AP projection. The patient had longstanding anaemia, koilonychia, glossitis and dysphagia, which recently increased in severity. UGI Endoscopy and biopsy confirmed the web and squamous cell carcinoma in the lower third of oesophagus. Remember, Plummer-Vinson Syndrome also called as Paterson-Brown-Kelly syndrome presents as a triad of dysphagia(due to oesophageal webs), glossitis and iron deficiency anaemia. These patients are at risk of developing squamous cell carcinoma of the oral cavity, hypopharynx and oesophagus.
