Plain Xray : Inferior rib notching, Left Ventricular Hypertrophy.
64 slict CT Angiogram Findings : 1. Narrowing of Descending Aorta distal to origin of left subclavian artery -Post Ductal Coarctation of aorta 2, Pre and Post stenotic dilation of Aorta as well as left dilated subclavian artery 3.Dilated aortic root 4.Left Ventricular Hypertrophy. 5.Prevertebral & chest wall collaterals.
DiscussionCoarctation of aorta can be either preductal(Tubular hypoplasia ) wth focal constriction of the aorta above the level of the ductus arteriosus and a lengthy narrowed segment of the aortic arch OR postductal( adult-type) with Focal constriction of the aorta just beyond the origin of the left subclavian artery and the ligamentum arteriosum Plain film (CXR) findings described are : Figure of 3 sign : contour abnormality of the aorta Inferior rib notching secondary to dilated intercostal collateral vessels seen in 70% of cases presenting in older children or adults left ventricular hypertrophy The rib notching involves 4th - 8th ribs occasionally involves 3rd and 9th ribs It does not involve 1st and 2nd ribs ( the associated arteries are branches of the costocervical trunk, and thus proximal to coarctation)
