STRESS FRACTURE OF TIBIA
Findings:
STIR hyperintensities reflecting edema noted in the ontramedullary portion of the diaphysis of proximal tibia. intracortical fracture line is noted in the medial aspect in both CT and MRI.
Discussion:
STRESS FRACTURE is common in tibia I t iccures due to Overuse injury, Change in training intensity or Active lifestyle in sedentary people. It has to be differentiated from Osteoid Osteoma . Thin slice (1-2mm) CT is used to detect the nidus or fracture line . MRI with STIR sequence identifies the marrow and periostela edema , whch are the earliest signs in stress fracture . key radiologiic signs of Stress fracture depends on the location: Osteal Bone: endosteal or periosteal callus formation without fracture line. Circumferential Periosteal reaction with fracture line through one cortex Frank fracture Cancellous Bone:: Flake like patches of new bone formation. Cloud like area of mineralized bone Focal linear area of sclerosis perpendicular to trabeculae MRI – early changes - periosteal , muscle and bone marrow edema.
