Answer for BIR CoW 08 Apr 2018
Acute Uraemic encephalopathy
Bilateral symmetric basal ganglia T2 / FLAIR hyperintensities with restricted diffusion and minimal swelling.
T2 / FLAIR hyperintense rim in both external capsules and medial boundaries of putamina (lentiform fork sign).
FLAIR / DWI hyperintensities in the left medial precentral gyrus , right superior frontal and right occipital regions.
Multi voxel MR spectoscopy revealed increased lactate peak and decreased NAA intergral values within the basal ganglia.
