Winner of CoW 31 July 2016
Pericardial cyst
Findings
CT-Fluid density mediastinal lesion compressing pulmonart artery and aorta. MRI-12 x 4.3 x 4.8cm well defined T1 hypo/ T2 hyperintense cystic lesion noted arising from the mediastinum causing indentation over the pulmonary artery and arch of aorta extending upto the superior mediastinum and inferiorly upto pericardial space along the anterolateral wall of left ventricle. The cystic lesion does not show diffusion restriction and causes compression of the left upper lobe apical and anterior segment.
Discussion
Pericardial cysts are uncommon benign congenital anomaly of the anterior and middle mediastinum. Clinical presentation In most patients are asymptomatic and discovered incidentally although occasionally patients may present with chest pain and dyspnoea. Pathology result from aberrations in the formation of coelomic cavities. They can occur as sequelae of previous pericarditis. The cyst wall is composed of connective tissue and a single layer of mesothelial cells, and usually contains clear fluid.
