Case Of the Week (COW) 04 May 2014
Answer:
ARACHNOIDITIS OSSIFICANS.
Findings:
Disc osteophytic complexes noted at L4-L5 level . intradural ossifications noted at the above site in CT. The cauda equina rootlets are also compressed . Syrinomyelia is noted in the lower dorsal spinal cord.DiscussionThis individual has history of laminectomy for disc herniation some years back. The constellation of findings such as Intradural Ossification associated with post inflammatory adhesion and clumping of nerve roots and syringomyelia associated with adhesions are consistent with diagnosis of ARACHNOIDITIS OSSIFICANS . Only few cases are reported in the literature about this uncommon entity.
