Answer:
Male pseudoprostate.
Findings:
Well defined lobulated predominantly solid mass, wedged between bladder neck & base of prostate, and showing internal vascularity . Seminal vesicles on both sides seen separate from the mass . Plane of cleavage between mass & prostate well made out.
Discussion:
We are dealing with a benign looking, predominantly extraluminal bladder base neoplasm. -Bladder tumors are broadly classified into epithelial(95% of cases )& non-epithelial masses -Because epithelial masses derive from the most superficial layer of the bladder wall, they often appear as polypoidal or irregular soft tissue structures of low- to intermediate-echo texture projecting into the bladder lumen from a fixed mural site. -In our case , the bladder mucosa was uninvolved. So we are dealing with a non-epithelial tumour. On Cystoscopy, extrinsic indentation of bladder base was noted ,alongwith normal looking overlying mucosa. List of differential diagnosis: Leiomyoma, the most common non-epithelial neoplasm comes first. Followed by its malignant counterpart – leiomyosarcoma. Neurofibroma, solitary fibrous tumor & lymphoma were added for completion sake. TRUS guided biopsy of pelvic mass done HPE diagnosis-leiomyoma.
