Case Of the Week (COW) 28 July 2013
Answer:
Sever`s Disease.
Findings:
STIR hyperintensities are noted in the calcaneal apophysis and postero lateral aspect of calcaneum with soft tissue edema – features suggestive of Calcaneal Apophysitis.
Discussion:
Mechanism of Sever`s Disease is thought to be due to traction apophysitis an repetitive microtrauma experienced during gait (similar to Osgood Schlatter's Disease) It occurs before closure of the calcaneal apophysis. It is a self-limiting entity that resolves with maturation and the closure of the apophysis MRI is used to diagnose and rule out disorders of the body of the os calcis (stress fracture, lytic lesion, osteomyelitis.
