Answer for BIR CoW 31 Jan 2021
Susceptibility Vessel Sign – Acute intra arterial thrombus
Findings
Gradient blooming noted in M3 segment region, reflecting thrombus
Discussion
Acute intra-arterial thrombus is paramagnetic due to the presence of a higher concentration of deoxyhemoglobin and clot retraction, which produces blooming artifact. 'Susceptibility Vessel Sign' is said to be positive when the diameter of a hypointense vessel exceeds the diameter of the contralateral artery on susceptibility-weighted imaging (SWI) images. The SVS is a highly specific sign of occlusive arterial thrombus, and is associated with larger infarct volume and poor clinical outcome Our case depicts SVS with acute right MCA infarct
