Answer for BIR CoW 16 July 2023
Orbital Subperiosteal hematoma
Findings
Evidence of T1 hypo and T2 hyperintense collection noted in subperiosteum of roof of left orbit causing proptosis showing no diffusion restriction Minimal vasogenic edema noted in bilateral basifrontal region Bilateral frontal hemosinus IMPRESSION : known case of trauma with multiple frontal bone fracture Feature suggestive of orbital subperiosteal hematoma (OSH) causing proptosis
Discussion
Orbital hematomas will be either intraorbital or subperiosteal in nature Mostly associated with craniomaxillary trauma Estimated incidence -0.3% MRI: well defined biconvex mass with curvilinear hypointense band I superior portion of orbit Acute hematoma : T1 intermediate signal T2 hyperintense chronic hematoma :T1 and T2 hyperintense MRI is the modality of choice in characterisation of hematoma
