Answer:
LEFT INTRACAVERNOUS EXTRADURAL EPIDERMOID CYST
Findings:
Heterointense extra axial cystic lesion noted in the left Parasellar region. Restricted Diffusion noted within the lesion. The lesion insinuates through the Meckel`s cave into the left cerebellopontine angle cistern, compressing and displacing the preganglionic segment of the trigeminal nerve posteriorly . The lesion encases the left internal carotid aretry flow void without causing luminal narrowing.
Discussion:
Primary Tumors of Cavernous sinus are either Interdural – Displace cavernous carotid arteries medially and do not encase. -Cranial nerve neurinomas, Epidermoid Tumor and Melanocytomas, Cavernous Angiomas. Intracavernous – Meningiomas –encase carotid artery. Parasellar epidermoid cysts are uncommon and these are commonly interdural . This is an unique rarely reported case of an Intracavernous Extradural Epidermoid Cyst . The clue is the encasement of the internal carotid artery and visualization of the outer dura , confirming the intracavernous extradural location.
