Answer for BIR CoW 10 Jan 2021
ZINNER SYNDROME
Findings
Left kidney not visualized Left ejaculatory duct obstruction causing T2 hyperintense collection proximal to the obstruction. Well defined T2 hyperintense cystic lesion of size 1.9 cm(AP)*2.8 cm(transverse)*3.5 cm(craniocaudal) noted in the left paramedian location in the region of left seminal vesicle
Discussion
Zinner syndrome - Mesonephric (wolffian) duct anomalies comprising of Unilateral renal agenesis, Ipsilateral seminal vesicle cyst and Ejaculatory duct obstruction Patient presents with perineal pain, recurrent prostatitis, hematospermia, painful ejaculation, and infertility, Maldevelopment of the distal part of the mesonephric duct results in atresia of the ejaculatory duct (leading to obstruction and dilatation of the seminal vesicle), while an abnormality in the ureteral bud leads to renal agenesis/dysplasia. DD of seminal vesicle cyst – prostatic utricle cyst,Mullerian duct cyst, ejaculatory duct cyst
