Answer for BIR CoW 07 Nov 2021
Testicular lymphoma
Findings
There is a hypoechoic mass in the left testis which shows increased vascularity. Moderate left hydrocele is also seen.
Discussion
Primary testicular lymphoma is an uncommon testicular tumour and accounts for 10% of testicular malignancies. It is the most common bilateral testicular tumour and most common testicular tumour in men over 60 years. Testicular involvement in lymphoma could be either primary or secondary. Diffuse large B cell lymphoma accounts for the majority of the cases. The most common clinical presentation is painless testicular swelling. Ultrasound may show diffuse enlargement of the testis with ill-defined hypoechoic signal and show increased vascularity. Orchidectomy and systemic chemotherapy is the preferred treatment.
